Local Care Partnership

LS2526 Primary Care Network is one partner in the LS25 & LS26 Local Care Partnership (LCP), A LCP is the term used in Leeds to describe our model of joined-up working to deliver local care for local people; working in and with local communities.

The LS25 & LS26 LCP covers the areas of Barwick-in-Elmet, Garforth, Swillington, Kippax, Methley, Oulton, Rothwell and all in-between. As well as the Primary Care Network, partners include statutory health and care partners, community groups, parish and local councillors and council services such as Active Leeds and Children’s Centres, who want to work together to improve the wellbeing of our local population.

Pre-covid the LCP had a strong focus on frailty and development the Planning Ahead Coordinator to support people living with frailty have discussions about their future care options.

During Covid we rebooted our joint priority to tackle obesity, and commissioned Healthwatch Leeds to understand what local people’s attitudes to diet and exercise had changed in the pandemic, to then better design how to support people.

From this the LCP has supported an Active Travel project and produced a local walking booklet, and working with citywide diabetes services to improve communications about health and wellbeing.

Active Leeds

Do you want to find out more about how to increase your physical activity?

Do you feel that you are struggling to become active again following the pandemic?

Active Leeds has various programmes aimed at increasing physical activity in a way that is manageable for you.

Active Life

At Rothwell Leisure Centre, Active Leeds deliver Active Life activities on Monday, Wednesday and Friday at 8:30-10am.

Active Life refers to activities designed for people 50+ to be active. An Active Life session consists of two activities. This could include an Exercise Class & mixed activities or 2 hours of a specific activity, such as Table Tennis, Walking Netball or Badminton. There is also an opportunity for a social meet up after your session.

Falls Prevention

In Garforth, Active Leeds deliver a free Postural Stability – Strength and Balance exercise referral programme. The sessions are for people age 60+ who are able to walk aided or unaided and are at risk of falls. They are designed to improve strength, balance and co-ordination for people with low mobility.

Age UK Leeds

Age UK Leeds is a local charity working in the community to support older people, their families, and carers. We want older people to be valued, to have their voices heard and to enjoy positive, independent lives.

We work together with organisations across the city to provide a range of services for older people including health and wellbeing activities, digital support and information and advice, including benefit entitlement checks.

Carers Leeds

We are working across all seven practices in our PCN to identify unpaid Carers.

These are people who care for a friend or family member due to their illness, disability, mental health issue or a substance misuse problem but are not paid to do it.

Sometimes these Carers are only children.

We are working to get all unpaid Carers identified by the practices. This is usually done by the filling out of a Yellow Card, which means we add a code to the Carers record so we can identify you.

The Yellow Card is then sent off to Carers Leeds and the Carer can be contacted by Carers Leeds if they wish to access more support.

If we know you are a Carer then we can make sure you get invited for your annual flu vaccination, we can try and make sure that if the person you care for has an appointment at the Surgery then you can have one at the same time if you need one.

We will also be offering all Carers an Annual Health Check, just to make sure that your health is good, and you are doing ok in other areas of your life. At this appointment we can refer you on for extra support if you need it.

We can also signpost you to groups in the area, and to websites that you may find helpful.

Clusters (Children/Families)

The LS2526 LCP is covered by three ‘Clusters’ which focus on offering and co-ordinating a range of Early Help/preventative services for schools and the children, young people and their families associated with those schools which pay into the Cluster. Each Cluster is typically managed by a Targeted Services Leader/Cluster Manager (see contacts below) on behalf of the local schools who predominantly fund services with some financial support from Leeds City Council Children’s Services and NHS/CCG. Whilst there are different levels of resourcing, each Cluster typically offers services around SEMH (social emotional mental health) for children and young people (e.g. by providing short term counselling in schools) and ‘Family Support’. With Family Support, Clusters will take a whole family (‘think family: work family’) approach and usually covers advice around parenting plus helps all member’s of a families’ needs to address a range of issues/needs effecting the quality of that families life including engaging with school/learning). Other issues that maybe covered include: housing, finance, substance/alcohol misuse, domestic abuse etc. Some Clusters may support the provision of some out of school activities including during school holidays. Clusters work closely with and can co-ordinate at local level (e.g. through an Early Help Plan) health provision and the wide range of Children’s and Families Services from Children’s Centres provision (for pre-school aged children) to Children’s Social Work Services and third/voluntary sector. Clusters can access a range of additional Family, adult and Therapeutic services such as through the Early Help/Family Hubs and intensive family work. Referrals to Clusters are via professionals/practitioners only.

Forward Leeds (Drugs & Alcohol)

Forward Leeds is the alcohol and drug service for Leeds. We help people choose not to misuse alcohol and drugs and reduce risk-taking behaviour through dedicated prevention, intervention and support.

We will support you to help achieve and sustain recovery from alcohol and drug misuse through a range of treatments. We offer a range of support and advice to people no matter how simple or complex their needs are.

Leeds Mental Wellbeing Service

If you’re feeling stressed, anxious, depressed or worried – Leeds Mental Wellbeing Service (LMWS) is here to help.

We can all experience problems in everyday life, and LMWS can offer support and resources to help you through these ups and downs. We can support you to manage everyday problems such as feeling low, anxious or stressed, or struggling with sleep. through our wide range of psychological support options, including phone support, instant-access online resources (Omnitherapy), group classes, and one-to-one ‘talking therapies’.

Kooth (Young people Mental Health)

An online mental health and emotional wellbeing service is available to young people (aged 10- 18) called Kooth (www.kooth.com). Kooth is a free, safe, anonymous online emotional wellbeing community that is accessible 24/7, 365 days of the year and provides access to chat sessions with BACP-accredited counsellors and qualified emotional wellbeing practitioners. All our chat sessions are text-based conversations that take place on the Kooth website, and last up to an hour. Chat is available from 12pm – 10pm Monday through Friday, and 6pm – 10pm Saturday and Sunday, providing a much needed ‘out of hours’ service. Kooth is accessible from any device that connects to wifi, allowing young people to seek support at a time and location that is suitable for them. Kooth is also so much more than just having chat sessions. Young people can get peer-to-peer support, as well as get involved with activities designed to improve emotional wellbeing and resilience. We allow young people to share their experiences safely, as everything contributed to the community be that a comment, a magazine article or a discussion forum is thoroughly pre[1]moderated by the online team before it’s published so that we can guarantee there won’t be any trolling, bullying or judgement – they also won’t be allowed to share any personal details with each other. You can also keep a mood-tracking journal, set and track goals and engage in live text-based discussions on the website. Kooth can be used as a sole means of counselling and support, or it can be used alongside another service as an additional tool of support if you’re already receiving counselling. There is no such thing as a problem that is too big or too small at Kooth. Our team reflects all communities, identities and walks of life, so no matter what issue you come to us with there is somebody at Kooth who can and will help.
